OPTIONAL PHOTOGRAPH / VIDEO AUTHORIZATION
IMPORTANT: This form affects legal rights. Please read it carefully before signing.
I am the parent or legal guardian of the child identified in this registration (“Child”), and I give permission for Child to participate in F.I. For Kids Co.’s (“F.I. For Kids”) live, instructor-led food and nutrition education classes and related activities (collectively, the “Program”). I represent that I have legal authority to sign this agreement for Child.
Selecting "Yes" during registration grants permission for photograph and video. Declining authorization, by selecting "No", will not affect Child’s ability to participate in the Program.
Photograph. I authorize F.I. For Kids to photograph Child during the Program.
Video and audio. I authorize F.I. For Kids to record Child’s image, voice, and likeness in video and audio during the Program.
Selecting "Yes" when registering Child, I grant F.I. For Kids a nonexclusive, worldwide, royalty-free license to use, reproduce, edit, display, publish, distribute, and otherwise use the resulting photographs, video, and audio recordings that I authorized above (the “Media”) solely to promote F.I. For Kids and its Programs, services, and related events, including on www.fiforkids.com and F.I. For Kids’ current or future social-media accounts, digital communications, printed promotional materials, and publications. This authorization includes use of my Child’s photograph, image, likeness, and voice for those promotional purposes and is intended to constitute prior written consent to the extent required by the Illinois Right of Publicity Act.
I understand and agree that neither I nor my Child will receive compensation, royalties, residuals, or any other consideration for F.I. For Kids’ authorized use of the Media, and I waive any claim to such payment.
F.I. For Kids will not intentionally identify my Child by full name in connection with the Media and will not sell the Media or my Child’s identity to a third party. I understand that social-media platforms and other public channels may permit third parties to view, share, or redistribute Media outside F.I. For Kids’ control.
This authorization is effective until I revoke it in writing. I may revoke it at any time by contacting F.I. For Kids at [info@fiforkids.com]. Revocation applies prospectively: F.I. For Kids will make commercially reasonable efforts to discontinue new uses and remove Media it controls from its website and social-media accounts within a reasonable time, but revocation does not require recall or removal of printed materials already distributed or Media reposted, archived, or otherwise controlled by third parties.
F.I. For Kids does not collect, capture, create, store, use, disclose, sell, or otherwise obtain biometric identifiers or biometric information from or about Child, including facial-geometry scans, voiceprints, or other information used to identify an individual through biometric means. Ordinary photographs and video recordings are not collected or used by F.I. For Kids for biometric-identification purposes.
I have read and understand this Authorization. I voluntarily provide the permissions I initialed above and agree to be legally bound by this Authorization.
